Village of Lynbrook Applications & Forms

Handicapped Parking Permit Application - Instructions and Application for a Parking Permit or License Plates, for People With Severe Disabilities NYS DMV MV-664.1 /664.3 (Adobe PDF). You must deliver the application to the Police Department in person. In addition you must have your driver's license or non driver ID number from your NYS DMV issued license or ID with you.
Block Party Application, Instructions and Requirements - (4 pages) Form should be mailed or delivered to the Lynbrook Police Department after filling it out. CAREFULLY READ ALL INSTRUCTIONS AND REQUIREMENTS!
Employment Application - (3 pages) For civilian positions with the Lynbrook Village Police Dept. (Adobe PDF). Form should be delivered in person to the Lynbrook Police Department after filling it out. Check the Employment page for job openings.
Alarm Permit - Required for all burglar, panic, holdup or fire alarm systems. Form should be returned to the Village Hall office after filling it out.

Automobile Accident Report Form - NYS DMV 104B - Use form MV-104B instead of form MV-104 to report an accident IF you did not file an accident report within 10 days of a reportable accident, as required by law, and your license is under suspension or you have received notice that your license will be suspended for failure to file an accident report.
